中文摘要目的了解呼吸重症监护病房铜绿假单胞菌(PAE)对20种抗生素的耐药性,并对其耐药性与抗生素用量间的关系进行分析。方法对我院2005年1月~2008年1月病房分离的PAE用20种抗生素进行药敏实验,并对RICU3年中7种抗生素的年用量与这些药物对PAE的耐药性进行统计分析。结果PAE最敏感的为哌拉西林/三唑巴坦,耐药率为12.5%,耐药率>90%的有氨苄西林、头孢噻吩、头孢西丁、头孢噻唔,头孢三代中最敏感的为头孢他啶,喹诺酮类的耐药率均<35%,其中环丙沙星耐药率最低;氨基糖甙类及亚胺培南的耐药率均逐年上升,亚胺培南耐药率46.5%。IPM、CAZ、AMK、CIP、氧氟沙星(OFL)对铜绿假单胞菌的耐药率和用药频度(DDDs)进行相关性分析显示,呈显著正相关(r=1,P<0.01),AMC、PIP的年用量与耐药率无相关性。结论RICU中铜绿假单胞菌耐药问题日渐突出,必须加强耐药检测,推荐将一种具有抗假单胞菌的β内酰胺类和一种抗假单胞菌的氟喹诺酮类或氨基糖苷类联合用药。; Objective To understand the respiratory intensive care unit of 20 Pseudomonas aeruginosa antibiotic resistance, the amount of antibiotic resistance and the relationship between analysis and the guidance of the rational drug use.Methods The data were retrieved from January 2005 to January 2008 in the ward of our hospital with PAE of 20 antibiotics sensitivity test, and RICU 3 years in the seven kinds of antibiotics, the use of these drugs on the resistance of PAE for the statistical analysis.Results PAE was most sensitive to piperacillin /tazobactam, and the resistance rate was 12.5%. The resistance rates more than 90% were ampicillin, cefalotin, cefoxitin, determination of well, and cephalosporins. Three generations of cephalosporins was most sensitive to ceftazidime, and the quinolone resistance rate was less than 35%, among them the lowest was ciprofloxacin resistance. Aminoglycoside and imipenem resistance rates increased yearly and imipenem resistance rate was 46.5%. IPM, CAZ, AMK, CIP, ofloxacin (OFL), using pseudomonas aeruginosa, and drug resistance frequency (DDDs), showed significant positive correlations (r=1,P<0.01). AMC, PIP, the amount of resistance were not related.Conclusion RICU resistance in pseudomonas aeruginosa become an increasingly prominent issue.We must strengthen the resistance testing, with a recommendation on the use of combination of anti-Pseudomonas β lactam, an anti-aeruginosa or the fluoroquinolones aminoglycoside combined.
