| 中文摘要 | 目的探讨冬虫夏草发酵制剂百令胶囊对慢性移植肾肾病(CAN)患者肾功能的作用效果,寻找防治CAN的有效方法。方法以CAN患者56例为研究对象,随机分为治疗组(A组,n=31),对照组(B组,n=25)。A组病人在原有免疫抑制治疗的基础上加用百令胶囊(2.0g/日,3次/日),前后共观察6个月,B组维持原治疗方案。于治疗前后分别检测两组病人的SCr、BUN、CCr、24h尿总蛋白(24hUpro);同时测定尿TGF-β1浓度以及尿视黄醇结合蛋白(retinolbinding protein,RBP)、尿β2-微球蛋白(β2-MG)。结果服用百令胶囊治疗6个月后,与治疗前比较,A组的SCr和CCr均有改善,差异具有显著性(P值均小于0.05),B组无明显改善;而BUN在两组中均无明显改善(P>0.05)。另外,A组的24hUpro和肾小管性尿蛋白组分(RBP及β2-MG)均明显低于治疗前(P<0.05或P<0.01),B组无显著性变化。A组的尿TGF-β1明显低于B组(P<0.05)。同时,A组中肾功能好转18例,稳定10例,恶化3例;B组中肾功能好转3例,稳定12例,恶化10例,两组比较,差异有显著性(P<0.05)。结论加用百令胶囊治疗CAN,可减少患者尿蛋白的排出,改善或稳定移植肾功能的恶化,延缓CAN的发展进程。; [Purpose] To investigate whether Bailing capsule (fermented agent of cordyceps sinensis) has any efficacy on renal function of patients with chronic allograft nephropathy (CAN). [Methods] 56 CAN patients were randomly divided into two groups. Patients in group A (n =31) were treated with immunosuppressive agents and Bailing capsule (2.0 g/day, three times a day). Patients in group B were treated with traditional immunosuppressive drugs. SCr, BUN, Ccr, and urinary protein in 24 hours (24 h Upro) of all patients were measured before and after treatment. Urinary concentration of TGF-β1, retinol binding protein (RBP) and β2-microglobulin (β2-MG) were detected at the same time. [Results] After treatment with Bailing capsule for six months, SCr and Ccr in patients of group A were significantly improved (P <0.05). No significant improvement of SCr and Ccr was observed in patients of group B. There was no significant change of BUN in patients of group A or group B (P >0.05). 24 h Upro, RBP and β2-MG were lower in patients of group A after treatment with Bailing capsule (P <0.05 or P <0.01), and TGF-β1 in patients of group A was significantly lower than in patients of group B (P <0.05). In group A, renal function was improved for 18 cases, stabilized for 10 cases, and worsened for 3 cases. In group B, renal function improved in 3 cases, stabilized in 12 cases, and worsened in 10 cases (P <0.05). [Conclusion] Bailing therapy could improve renal function of CAN patients, and retard CAN progression. |
