中文摘要目的:探索犬膀胱全切后以回肠构建全尿道、膀胱术的实验方法。方法:实验于2004-12/2005-07在吉林大学药学院生物工程研究室完成。体质量13kg的雄性京犬12只,术前禁食24h,以30g/L戊巴比妥钠按1mL/kg剂量对犬麻醉。取腹部正中切口,切除膀胱及尿道。在距回盲部约10cm处取一段30cm带血供的回肠袢,恢复肠道的连续性。取25cm回肠沿着肠管系膜的对侧缘纵向剖开,将剖开的肠管对折缝合成袋状,并将两侧输尿管种植于回肠新膀胱的适当位置。取5cm回肠剥离黏膜层并缝制成管状,构建尿道,在回肠新膀胱的底部戳孔与其缝合。连接在膀胱及尿道的原位,术中静点氧氟沙星100mL(含氧氟沙星0.2g/L)、葡萄糖注射液250mL、生理盐水250mL、缝合术后刀口,并静点氧氟沙星0.2g/d,连续给药7d,1周后正常进食、继续饲养1个月,观察动物排尿情况,行X射线影像学检查。结果:9只犬进入结果分析,另3只由于麻醉过度死亡。术后1个月观察实验动物犬,进食正常,排尿正常。术后4周X射线造影检查显示:贮尿囊显影良好,无梗阻;回肠替代膀胱充盈,肠黏膜替代尿道再生状况良好。结论:建立的犬膀胱全切原位回肠代膀胱、尿道术的实验方法,可做为组织工程膀胱、尿道移植研究的模型。; AIM: To explore the empirical method of the replacement of bladders and urethras by orthotopic cyno-ileum. METHODS: The experiment was conducted in the bioengineering laboratory, School of Pharmacy of Jilin University between December 2004 and July 2005. Twelve male dogs of 13 kg were fasted for 24 hours before the operation; the abdomen of the dogs were cut off to remove the bladder and urethra after anesthetized with 30 g/L pentobarbital at the dose of 1 mL/kg. A piece of 30 cm ansa interstinalis with blood was harvested at the distance of 10 cm away from ileocecal junction to recover enteric continuity. Ileum of 25 cm was taken to cut the canal thecal laterally, and then the intestinal canal was sewed inside out to a bag shape. Two sides of the ureter were implanted into the suitable place of the new ileum bladder. Ileum of 5 cm was taken and the stratum mucosum was discard and sewed as a canal to construct a urethra, which was sewed with the new bladder after its bottom was poked as porous. Then it was located to the orthotope of bladder and urethra. During the operation, 100 mL ofloxacin containing 0.2 g/L ofloxacin, 250 mL glucose solution (5%) and 250 mL normal saline were intravenously injected; the blade was sewed after operation and injected intravenously 0.2 g per day ofloxacin for 7 days. One week later, the dogs were given normal diet for 1 month to observe the urination and carry out X-ray imaging examination. RESULTS: Nine dogs were involved in the result analysis, and the other 3 died of over-anesthesia. One month after the operation, the dogs took food and urinated normally. The X-ray contrast examination showed that the allantois was developing well with no obstruction; the replaced bladder by ileum was full, and the regeneration state of the intestinal mucosa replacing urethra presented a good condition. CONCLUSION: The methods of replacing bladders and urethras by orthotopic cyno-ileum could serve as the models of the study on bladder tissue engineering and urethra transplantation.; 国家高技术研究发展计划(2004AA205020); 吉林大学创新工程项目资助~~
